Medicare covers Eversense 365 for people taking insulin to treat diabetes3 . Other patients might also qualify.

Coverage Details

Insurance providers typically limit CGM coverage to people who take insulin.

If you’re on insulin, Eversense 365 is covered by most commercial health plans.1 This includes Aetna, Cigna, Elevance (formerly Anthem), Humana, United Healthcare, most Blue Cross Blue Shield plans - and many others. If your plan does not cover, we encourage you to contact your insurance provider to request an exception.

If you pre-qualify, your Eversense Expert can help with a free benefits check. With a commercial insurance plan, you may also be eligible for our Eversense PASS® programs for further savings!

Medicare covers Eversense 365 CGM for people with diabetes who take insulin. People who are not on insulin but experience low blood glucose events may also qualify.
To qualify for a Eversense 365 CGM with Medicare, you must:

  • Have diabetes
  • Take insulin or have a history of low blood glucose
  • Have been trained to use a continuous glucose monitor as prescribed by your doctor
  • Make routine visits with your doctor

Yes, insurance may cover both the product (sensor and transmitter) and the insertion and removal procedures. The Eversense® E3 Continuous Glucose Monitoring (CGM) System is indicated for continually measuring glucose levels for up to 180 days in persons with diabetes age 18 and older. The system is indicated for use to replace fingerstick blood glucose (BG) measurements for diabetes treatment decisions. Fingerstick BG measurements are still required for calibration primarily one time a day after day 21, and when symptoms do not match CGM information or when taking medications of the tetracycline class. The sensor insertion and removal procedures are performed by a health care provider. The Eversense E3 CGM System is a prescription device; patients should talk to their health care provider to learn more.
